Direct Primary Care
You're probably used to accessing medical care by paying for insurance coverage, often for a high price. You sort through your insurance company's list of in-network doctors, arrange for an appointment and hope your insurance company covers the services you need.
Direct primary care (DPC) is different. You won't have to deal with a guessing game every time you come to the office. Direct Doctor Care provides exceptional medical services for families in Middleton, Idaho, so you can count on us to guide you through the direct primary care model.
If you become as one of our patients, you'll participate in membership-based healthcare services. You'll pay an affordable monthly membership fee that will allow you to cut insurance out of the equation and pay reduced out-of-pocket costs.
Membership Enrollment
YOU SHOULD KNOW:
- Monthly membership dues will be charged automatically from your credit card, debit card, or directly from your bank account.
- You can choose our yearly membership and save 10% by pre-paying for the entire year.
- We charge a one time $75 registration fee per individual/family.
- The price for families maxes out at $225 per month.
- If you decide you no longer want our services and then re-enroll you will be charged a $200 re-enrollment fee.
- You may cancel at any time. No refunds are allowed.
We do not deny membership for pre-existing conditions or lack of insurance coverage. Once you complete your medical history form(s) we will contact you if we have any questions or concerns about our physician's ability to properly care for you given your particular healthcare needs. If for any reason your membership is not accepted, you will receive a full refund.
